We are in need of data for our Control Group. This Form is a 2 hour questionnaire that dives deep into all the parts of you. This data allows us to assess the Starting Point (Point of Origin) of those who enter The Healing Garden’s Triadic Healing. 

While this is — very much — optional, we do ask that you please fill out the form so we can improve Triadic Healing and ensure it is always at its highest quality for what the people need. 

This form is equivalent to “The Intake” form.
